Tretinoin is able both clinically and histologically to partially and gradually reverse photodamage in skin. Retinoic acid repairs photodamage and prevents further UV damage. Simutaneous use of sunscreens enhances. The inherent mechanisms causing acne and dilated follicles are also decreased by the consistent use of retinoic acid.

Clinical response to topical tretinoin results from improvement in both epidermal and dermal components of photoaged skin. Long-term studies show that noticeable skin changes with tretinoin treatment result primarily from the increase in dermal collagen, decrease in abnormal elastin, decrease in abnormal clumped melanin, and repair of the dermal-epidermal junction. Dyspigmentation, surface roughness, and fine wrinkles demonstrate the most consistent and significant improvement.
